The product

TakeCare brings appointments, telehealth, messaging, records, and follow-up into one patient experience. The product also gives care teams the operational tools needed to keep that experience moving.

The work was not only about building screens. Every patient action needed a dependable state, a clear next step, and a corresponding workflow for the people delivering care.

The challenge

Healthcare journeys cross several moments that are easy to design separately but difficult to operate as one system. Booking affects availability. Telehealth depends on appointment state. Messages and records need the right access. Reminders need to arrive at the right moment.

The challenge was to make those dependencies feel calm to the patient while keeping the underlying workflow visible and manageable for care teams.

Designing around the complete care journey

The patient experience was shaped around what happens before, during, and after care. That meant treating appointments, calls, messages, records, and reminders as connected stages rather than isolated features.

Clear states helped the application show the right information and actions at each moment, while the backend protected the rules behind those transitions.
